Apply for Home Care Package

You can contact My Aged Care on 1800 200 422 or simply go to My Aged Care website for assistance. They will put you in touch with your relevant Aged Care Assessment Service where the process will commence.

If you are under 65 years of age, go to www.dhs.vic.gov.au to learn what assistance may be available for you.

Needs Assessment

The Aged Care Assessment Service (ACAS) will access your needs and determine the appropriate level of care.
There are four levels of home care packages with a maximum contribution from the government associated with each level of needs

  • Home Care Level 1 – basic care needs
  • Home Care Level 2 – low level care needs
  • Home Care Level 3 – intermediate care needs
  • Home Care Level 4 – high care needs
The actual amount paid by the Government for each package (Government subsidy) will depend on the financial assessment in the next step to determine your contribution.

Financial Assessment

The government (Centrelink) assesses your financial situation and based on that assessment you may be required to contribute to the cost of your care.

Your contribution (income tested fee) plus the Government subsidy adds up to MAXIMUM CONTRIBUTION appropriate to your level of care.

If you’re not assessed as eligible, you can access privately funded services through Sequel at any time.
